Mail completed entries, along with a list of models entered(Name, Breed, Sex, Divisions entered, If this is not followed the model will not place. And If applicable their Region 1 tag Saver numbers) to: If you do not have a Tag Saver number please let the showholder know and a number will be given to you for this show.
By signing this form and attending, I hereby release Dawn Clute, Missoula 5 Valley Model Horse Show, Quality Inn, City of Missoula harmless from any loss or damage or theft to my person or my personal property during show activities. The Show holder, Quality Inn, and City of Missoula are not responsible for lost items, damage of theft to any items. Signed entry form is required from each shower before the show starts. Signature .Date If entrant is a minor, a Parent/Guardian must sign. Please print name signed. My Show Hall Space is limited, if possible please share a table with someone.
IF YOU HAVE SENT IN YOUR ENTRY BUT FIND YOURSELF UNABLE TO ATTEND THE SHOW, A REFUND OF 90% OF THE TOTAL FEES PAID WILL BE GIVEN, IF THE SHOWHOLDER IS NOTIFIED AT LEAST 2 WEEKS PRIOR TO THE DATE OF THE SHOW. CANCELLATIONS AFTER THIS WILL BE GIVEN A 50% REFUND OF ALL FEES PAID. NO EXCEPTIONS!!!!
The entry fee's pay for the show hall rental fees, ribbon/rosette/trophy fee, As well as for some of the other prizes at the show, NAMHSA menbership fee, printing, mailing. and other costs involved in putting on the show.
If you are going to donate to the auction or as a raffle item, Please email me about it. All donators will receive free add space in the show packet and on my web site.
